We have read the letter written by Dr. González regarding the possible effect of tamsulosin on iris pigmentation. We find the association very interesting and plausible as a mechanism since sympathetic innervation has been well described to affect iris pigmentation. However, we agree with Dr. González that their study is not designed to prove a cause and effect relationship between tamsulosin use and iris pigmentation and could just represent selection bias or random chance in their limited population. Although we have noticed an increased incidence and severity of intraoperative floppy iris syndrome (IFIS) in lightly pigmented irises, we have not observed a clinical association between tamsulosin use and a decrease in iris pigmentation.
