Floppy Iris Risk Factors
We read with great interest the article by Chatziralli and Sergentanis entitled, “Risk Factors for Intraoperative Floppy Iris Syndrome: a Meta-analysis.” The authors found a hierarchy concerning α1-blockers in floppy iris syndrome (IFIS) occurrence, with tamsulosin having a 40-fold greater than the other compounds of the same family, i.e., alfuzosin, doxazosin, and terazosin. In addition, regarding the collateral factors, they concluded that “ . . . hypertension, but not diabetes mellitus, emerged as a risk factor for IFIS.” Consequently, a question arises: what is the weight of the therapy and that of the disease; in other words, is the disease per se, (hypertension) that leads to IFIS, or rather, is the therapy responsible for its occurrence?
